Hospital appointments

If you attend hospital and are asked for the name of your G.P. please make sure that you give the hospital staff the full name of your doctor and explain that you are registered with Leadgate Surgery.

Booking an Ambulance

Our receptionists can book patient transport to hospital appointments for those patients who are eligible. Before an ambulance is booked we must go through the assessment criteria with you. This is a series of questions created by the ambulance service to determine if a patient fits the criteria for ambulance transport. We must ask all patients requesting transport, or their carers, to complete the questionnaire. After answering the questions the receptionist will then be told if your request has been accepted. If you are unhappy with the decision to refuse the ambulance transport you can contact the ambulance service on 0191 430 2000.
