How to register

We welcome patients from within the Practice area around Leadgate, you can check if you are in the practice area by using the Map below.


 

If you wish to register as a patient, please call in at the Surgery where you will be asked to complete a couple of forms. You must provide two forms of identification, including one photo ID and one with a current address from a financial institution (utility bills are not acceptable).

We will then arrange an appointment with one of our nurses for a “Registration Check” so we can collect up to date information about you. Then, if necessary, you will be given an appointment to see a doctor.

Children under five do not need a registration check.

As part of a government scheme, from 1st April 2015, all patients are allocated a named GP who is responsible for your overall care at the practice although you are still free to see any GP you choose.  A member of the practice team will be able to let you know who your GP is.  Should you have a preference for your named GP, please speak to a Receptionist who will help with your request.
